What to bring

  • Your Ontario health card and one piece of ID.
  • Please arrive 15 minutes before your appointment.
  • Details of any other health insurance you have.
  • Any test results, records or images your doctor asked for.
  • Any logs your doctor asked you to keep, for example blood pressure, blood sugar, pain or headache logs.
  • The name and phone number of your emergency contact, and your current address and phone number.
  • A list of other health care providers you saw recently, why you saw them, and what they told you.
  • All the medicines you take, including vitamins, supplements, herbal products and medicines you buy without a prescription.
  • Any changes to your medicines since your last visit, including changes made by a specialist.

Cancelling or changing an appointment

If you cannot come, please cancel at least 24 hours before your appointment. If you miss an appointment, cancel with less than 24 hours’ notice, or arrive late, you may be charged a missed-appointment fee: $50 for a regular appointment, or $100 for an annual check-up, an IUD insertion or removal, or a consultation. You will need to book a new appointment.

If you arrive late, your physician may not be able to see you, and you may need to book a new appointment for another time.

Missed-appointment fees are not covered by OHIP. The optional block fee covers the $50 fee for a regular appointment. See our fees for uninsured services.

To cancel or change an appointment, please call your clinic.

Read our clinic policy for the full rules on booking, missed appointments and fees.

AI scribe

Some of our doctors use an AI scribe. It is a note-taking tool that helps document your visit, so your doctor can focus on you instead of typing notes.

  • It is your choice. Your doctor will ask for your consent before using it. You can say no, or ask your doctor to stop at any time. This will not change the care you receive.
  • Your doctor stays in charge. The AI scribe writes a draft summary of your conversation. It does not make decisions about your care. Your doctor reviews the note and makes sure it is accurate before it goes into your medical record.
  • Your information is protected. Your doctor only uses AI scribes that follow Ontario’s Personal Health Information Protection Act (PHIPA). Your information is encrypted, kept in Canada and not used for any other purpose. The AI scribe does not keep recordings of your visit.

If you have questions about the AI scribe, including which one your doctor uses, please ask at the start of your visit.
